"Affirmation of Consultation"
letters were mailed to Milwaukee non-public schools on October 30,
2008. Milwaukee non-public schools need to submit all three of
the items listed below by December 19, 2008 to be eligible to
participate in ESEA Title programs through MPS in the 2009-2010
school year. Since an official student enrollment count must take place
on December 4, 2008, documents should not be submitted before that date.

Milwaukee non-public schools needed to submit all three of the items listed below by December 14, 2007 to be eligible to participate in ESEA Title programs through MPS in the 2008-2009 school year.
Items1 and 2 above were to be submitted via mail or hand delivery to: MPS Attn: Room 253, #11; 5225 W. Vliet Street; MilwaukeeWI 53208.
Item 3 was to be sent in any universally recognizable data format as an email attachment to: valentec@milwaukee.k12.wi.us. When item 3 was emailed, senders got e-mail confirmation regarding the receipt of all Affirmation of Consultation documents. Other Affirmation of Consultation documents are provided below.

Certified "Affirmation of Consultation" letters were mailed to Milwaukee non-public schools on November 9, 2006. Milwaukee non-public schools needed to submit all three of the items listed below by December 15, 2006 to be eligible to participate in ESEA Title programs through MPS in the 2007-2008 school year.
1. Affirmation of Consultation - This document provides
or updates school contact information for local and state school
databases and allows non-public schools to request Title program
participation for the coming school year. Applications for Title
program participation must be submitted each year. This document must
be signed by a private school representative. 2. Enrollment Summary - This information provides
enrollment information that allows MPS to calculate ESEA funds to be
reserved for non-public schools and generate individual school
allocations. It also assists in compiling Milwaukee student census
data. This document must be signed by a private school representative. Enrollment
counts must be done for students enrolled on November 30, 2006. The
information can not be submitted before then. 3. Student Information - Names, addresses, phone
numbers, birth dates and parent/guardian names for all students counted
in the school Enrollment Summary must be provided to document numbers
submitted. The Excel form attached may be used to facilitate
submission, but schools may use any typed format that provides the
required information. Enrollment counts must be done for students
enrolled on November 30, 2006. The information can not be submitted
before then.
