About some "compressed" files you will find
here:
Some files available here are "compressed" to allow quicker and
more reliable downloads over the internet. PC files are .exe and Mac
files are .sea files that will uncompress themselves when clicked
after downloads are complete. After you have uncompressed one of
these files, a new "uncompressed" version of it will be available to
you. You can then throw the compressed version away and click on the
uncompressed version to use it.

Title IA Parent/Guardian Consent
Information

Title IA Targeted Assistance School Parent/Guardian
Permission to Participate Template for Public Schools. Any student
who participates in a Title IA targeted assistance program must have
parent consent to do so. This is a Word document. A non-public school
template is available at http://www2.milwaukee.k12.wi.us/title_i/Web_Pages/non-public.html.
Current consent forms should be kept on file in school office.

Title I Eligibility Report Information
A new Title I Eligibility Report form for Targeted Assistance
schools is now available. Download the version designed for your
computer.

2005-2006 Title I Eligibility Report:
Eligibility Reports provide information about students
receiving Title I services. Public Schools designated as "Targeted
Assistance" schools and non-public schools with students that recieve
services for their Title I eligible students are required to submit
this report. The reports folder that can be downloaded below contains
compressed FileMaker Pro runtime documents that allow digital
Eligibility files to be maintained. Non-public schools need only submit
paper eligibility forms (available below) to their Title I service
providers. Download both the Worksheet and the directions.

About FileMaker Pro Runtime Documents
Eligibility Reports come in a compressed folder that contains
both a FileMaker Pro database to use (called "SendToTitleI") and a
"runtime" helper application to allow you to use the database on your
computer.

What to do with completed Eligibility
Reports
Public Sshools designated as Targeted Assistance Schools and
all Non-public schools are required to compile information about
eligible students using the Eligibility Report Worksheet and Quick
Eligibility Report Directions and Ranking Criterion available as
downloads above. Completed eligibility work sheets should be turned
in per arrangements made by site administrators. In non-public
schools, Title I service providers will work with school staff to
identify those eligible students with the greatest academic needs who
will get Title I services. Public schools may contact the Title IA
office at 475-8009 for assistance in this process. Services may not
be extended to students not identified in this process. Check for
accuracy and completeness before turning in reports:
- Is Demographic Information complete for all eligible
students?
- Every student who is listed as eligible for Title I service in
the nonpublic, Targeted Assistance programs needs to have one or more
of the eligibility criterion checked.
- All students who are eligible must have a Title I rank of
1, 2 or 3 (ranks of 1 indicate students with greatest needs).
- Students should be getting service based upon their Title I
ranks. No student with a lower rank should get services unless all
students with higher ranks are also getting service.
- Staff members at non-public schools should not check academic
service area fields since determinations about which students will
get service will be made when all of the initial screening work is
done. Public schools should check service area fields to
indicate areas where students are getting Title Ia support.
- Is report header information complete and accurate? (E-mail and
FAX numbers are not required.)
Once schools have identified eligible students and decided, in
consultation with Title I personnel, upon those to be served,
eligibility information should be turned over to local Title I
personnel for entry into electronic database. Academic service
areas should be indicated for the students who are being served
by Title I in non-public school programs (Local Title I personnel are
responsible to see that it is completed and transmitted to the Title
I office.)
Title I law allows service to eligible students enrolled in
non-public "targeted assistance" schools, but not to the schools
themselves. Care should be taken to ensure that Title I services are
not extended to too large a percentage of targeted assistance school
students. Contact Edward Valent, Title Program Supervisor at the MPS

limitation.
Send completed Eligibility Report to Title I office:
- Attach the <SendToTitleI> document to an email and send to
Title I Supervisor, Edward Valent at valentec@mail.milwaukee.k12.wi.us,
(receipt will be confirmed via return email) or
- Copy the <SendToTitleI> document to a floppy disc or CD and
submit it by hand or MPS interoffice mail to the MPS Title I office at
at 5225 W. Vliet Street; Room 253, Office 11; Attention Mr. Valent or
by US Mail to Title I, Room 253, Office 11; Attention Mr. Valent; P.O.
Box 2181; Milwaukee, WI 53201-2181
- Submission of electronic version of eligibility database file is
required. Paper submission is not sufficient.
Eligibility Report Databases are due in the Title I office
soon after the start of each school year. They should be kept current
at local schools.
